Position Summary Under the direction and supervision of a Qualified Health Professional, the Targeted Case Manager (TCM), will provide comprehensive case management services to patients who have histories of behavioral health needs specific to the need for induction of Buprenorphine (Suboxone). The TCM will coordinate, address, and connect patients to resources in support of their long-term recovery.
Position Description
Coordinate case management services for behavioral health MAT patients
Facilitate referrals to service providers as appropriate
Support PRN in maintaining contact with patients via outreach phone calls and letters
Advocate for patient care to reduce barriers to accessing services
Document and bill all interaction recorded in the electronic health records (EHR)
Adhere to confidentiality policy and HIPAA protocol as related to patient’s information/interactions
Attend all agency staff meetings and annual in-service as required or mandated.
Maintain good working relationships with all agency staff, patients and visitor
Position Qualifications
Bachelor’s degree in Human Services, Social Work, Psychology, Counseling, or related field
Prior experience working with target populations
Experience and knowledge of Microsoft systems and electronic health records (EHR)
Excellent written and oral communication
Ability to speak, read and write the English language
Bilingual English/Spanish a plus
Job Setting
Must possess sight, hearing and digital senses or use prosthesis that will enable the essential functions of the job to be completed.
