Summary

Pay Class: First shift

Hours Available: 7:30am to 3pm

Site Location: Southside/Baymeadows area, Jacksonville, Florida 32256

Rate of Pay: $15.00

Behavior Focus Day Program Center

Scope of Role

Provide direct support within a single program or unit, typically supporting 4‑6 individuals in residential settings, or up to 10 individuals in periodic programs.

Key Responsibilities

Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.

Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.

Services and Supports

Provide direct support per service plan.

Accurately document progress and activity; maintain confidentiality.

Make cost‑effective purchases within the individual’s budget and according to their choice.

Maintain professional relationships with families, guardians, and case managers.

Report abuse, neglect, medical, behavioral incidents to internal and external standards.

Respect and preserve the rights of persons served, practice universal precautions.

Health Care

Accompany individuals to medical appointments as needed.

Administer or support self‑administration of medication; document deliveries.

Monitor health concerns; communicate with nurse or supervisor; call 911 in emergencies.

Follow procedures for medical equipment; report malfunctions.

Prepare or support individuals in preparing food; adhere to special diets.

Employment Responsibilities

Attend orientation and ongoing training; participate in monthly staff meetings.

Work scheduled hours; arrive on time; stay until replacement arrives.

Notify supervisor promptly if unable to work.

Collaborate as a team and communicate openly with respect.

Maintain a safe workplace and report hazards.

Maintenance

Transport individuals safely; obey traffic laws; report accidents.

Perform daily and seasonal housekeeping; report hazards.

Make minor repairs, change light bulbs, maintain yard safety.

Check water temperature; participate in safety drills.

Other

Perform other duties as required.

MPA = Mobile Punch Authorized for timekeeping.

None

Minimum Knowledge and Skills Required

High‑school diploma or equivalent preferred.

Six months of experience in human services preferred.

Must be 18 years of age.

Working knowledge of computers.

Certificates, Licenses, and Registrations

Valid driver’s license in good standing.

Car registration and vehicle insurance if providing transportation.

All state‑required training and certification completed on time.

Maintain valid license and meet insurance requirements; approved to drive by management.

Work Environment

Work at program location; may accompany individuals into community.

Physical Requirements

Heavy work: lift up to 100 pounds occasionally, 50 pounds frequently, 20 pounds constantly.

Additional Details

Cooperate with licensee and Department of Human Services staff in any inspection or investigation.

Valid driver’s license in good standing in the state of New Jersey; or state of residence if commuting.

High school diploma/Equivalent required.

All employees hired after 6/15/2018 submit to drug testing pre‑employment and on a random and for‑cause basis.

Criminal background checks and registry checks per state requirements.

Complete all required training and demonstrate proficiency.

At least 18 years of age.
