Overview
FLSA Status: Non-Exempt
Location: Holy Family Medical Center
Shift: Full-Time, 12-Hour Shifts (Days or Nights)
Description: The Licensed Practical Nurse (LPN) provides direct nursing care to patients in accordance with professional standards and established policies and procedures. The LPN collaborates with the interdisciplinary care team to support patient treatment plans, promote positive patient outcomes, and ensure a safe, therapeutic environment. This position requires strong clinical skills, effective communication, and a commitment to delivering high-quality patient care.
Responsibilities
Provide direct patient care under RN or provider supervision.
Support development and implementation of patient care plans.
Administer medications and treatments per protocol.
Monitor and document patient condition; report changes promptly.
Assist with assessments, ADLs, mobility, and safety needs.
Maintain accurate and timely EHR documentation.
Educate patients and families on care plans and instructions.
Collaborate with interdisciplinary team members to ensure coordinated care.
Adhere to all regulatory, safety, and infection control standards.
Qualifications
Current and valid state license as a Licensed Vocational Nurse.
Current BLS certificate upon hire and maintain current.
IV certification upon hire; Blood Withdrawal (BW) certification preferred.

Pay Transparency Competitive compensation estimated at $31.00 to $40.00 per hour. Exact starting compensation will be determined at the time of selection based on skills, experience, education, credentials, and licensure.
