About the Role
The Registered Nurse (RN) plays a critical role in delivering comprehensive nursing care to patients across the home healthcare setting. This position focuses on assessing patient health status, developing and implementing individualized care plans, and providing direct patient care to promote optimal health outcomes. The RN advocates for patients' needs and educates them and their families on managing health conditions and treatments effectively. Additionally, the role involves monitoring patients using medication management and managing wound care to prevent complications. The ultimate goal is to ensure high-quality, compassionate care that supports patient recovery, comfort, and well-being. Minimum Qualifications
Current and valid Registered Nurse (RN) license in the practicing New York State. Completion of an accredited nursing program (Associate's or Bachelor's degree in Nursing). Proven experience in patient assessment and direct patient care in home care or long-term care settings. Basic proficiency in medication management monitoring and wound care management. Strong communication skills to effectively educate and advocate for patients. Preferred Qualifications
Bachelor of Science in Nursing (BSN) degree. Certification in wound care. Experience working in home care settings with home medical equipment. Familiarity with electronic health record (EHR) systems. Responsibilities
Conduct thorough physical and patient assessments to evaluate health status and identify care needs. Provide direct nursing care including wound care, medication administration, and monitoring of vital signs using telemetry. Advocate for patients by coordinating with interdisciplinary teams to ensure patient-centered care and address any concerns. Educate patients and their families on disease management, home medical equipment usage, and health maintenance strategies. Document patient care activities accurately and maintain compliance with healthcare regulations and standards. Skills
The required skills such as patient assessment, physical assessment, and direct patient care are essential for evaluating patient conditions and delivering appropriate nursing interventions daily.
