Limited patient census, allowing for meaningful time with patients

Key Responsibilities

Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in the home environment

Determine follow‑up assessments according to patient needs and best practice guidelines

Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care

Assess health status by obtaining relevant health and medical history

Perform a physical exam based on the age and patient history as per the Standard Care Arrangement with a collaborating physician

Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient’s clinical problems and healthcare needs

Perform diagnostic or therapeutic interventions consistent with certification, law, and state medical privileges

Prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency

Develop a treatment plan in collaboration with the patient and caregivers

Refer patients to physicians as appropriate for consultation or for specialized health resources and treatment

Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas

Provide patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits

Hold case conferences with families when appropriate to advance the care plan

Meet minimum productivity requirements based on practice standards

Required Qualifications

Master's or doctoral degree as a Board‑Certified Nurse Practitioner

Current state license as a Nurse Practitioner

Nursing skills as defined by accepted nursing standards

Ability to sit, stand, bend, lift, and move intermittently

Must be able to lift at least 35 lbs and bear the weight of an average adult effectively

Current and valid driver’s license

Reliable transportation and valid automobile insurance coverage

Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled nursing facility settings

Demonstrates skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles

Ability to handle multiple priorities, documentation requirements, and deadlines

Desired Qualifications

Post‑acute or Nurse Practitioner experience preferred

Adult, Family, or Gerontology Nurse Practitioner certification preferred

Experience with geriatric patients preferred

Pay Range $100,000–$120,000 USD
