R25095596**Summary****Job Description:****Baptist Health Medical Group is looking for a Transfer Center/Triage RN to join their team. This is a remote work position that requires residency in KY or IN**The RN (Medical Call Center) is responsible for providing professional nursing care through telephonic interventions focused on the patient and caregiver as a whole. This role emphasizes healing and wellness by managing patient needs through readmission management, nurse triage, service referrals, telehealth monitoring, and coordinating transfers. The RN demonstrates a commitment to Baptist Health's quality standards, organizational values, and safety protocols.**Essential Duties:**
• Assess patient and caller needs through telephonic interactions for areas such as readmission management, triage, and service referrals.
• Provide telehealth monitoring to evaluate patient conditions and follow up on care plans.
• Facilitate external patient transfers and bed placements both internally and externally for assigned facilities.
• Develop and implement individualized plans of care based on patient assessments and needs.
• Coordinate issue resolution for patients, ensuring timely and effective responses to concerns.
• Complete calls within established guidelines, documenting interactions accurately.
• Crosstrain in various roles within the call center to provide backup assistance as required.
• Build collaborative relationships with healthcare teams to achieve patient care goals.
• Participate in case discussions and team meetings to enhance care coordination.
• HIPAA regulations.
• Solicit and utilize patient feedback to improve care delivery and address any service gaps.
