Empath Home Health, a member of Empath Health, is seeking a compassionate
PRN Home Health SLP
to provide treatment to patients in their homes in the
Venice, Nokomis, and surrounding area
to improve sensory‑motor development, perception, memory, language, and cognition. In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.
Locations:
Home health visits available in
Venice, Nokomis, Laurel, and surrounding areas.
It is very flexible - you choose where you want to work!
Schedule:
PRN; 3-5+ home health visits per week

What You’ll Do
Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
Develop a plan of care in consultation with the physician and other care team members.
Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
Instruct patients and families on using special educational strategies and techniques to improve sensory‑motor development, perception, memory, language, and cognition.
What You’ll Need
Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
Home health care experience preferred.
Experience using Homecare Homebase (HCHB) EMR is a plus!
