Title: PRN Home Health SLP Speech Language Pathologist – Empath Home Health - Venice, Nokomis
Locations: Venice, Nokomis, Laurel, and surrounding areas.
Schedule: PRN; 3-5+ home health visits per week

What You’ll Do
Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
Develop a plan of care in consultation with the physician and other care team members.
Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
Instruct patients and families on using special educational strategies and techniques to improve sensory‑motor development, perception, memory, language, and cognition.
What You’ll Need
Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
Active Florida SLP license.
Home health care experience preferred.
Experience using Homecare Homebase (HCHB) EMR is a plus!
