Care Management Navigator WOW, 7a-7:30p Weekends – Norton Healthcare
Job overview This is a WOW (Work on Weekends) position. Schedules for WOW employees include every weekend (Nights - Friday & Saturday, Days - Saturday & Sunday). Employees in designated WOW position earn premium pay in addition to their competitive hourly rates and shift differential.
Responsibilities
Collaborate with multidisciplinary healthcare team to assess, plan, coordinate, monitor, and evaluate patient care, treatments, and services.
Coordinate discharge plan and assist in monitoring patient outcomes.
Provide ongoing support and expertise to the healthcare team and advocate for the patient throughout the stay.
Communicate and coordinate with the patient and/or representative and other team members.
Apply nationally recognized criteria sets to promote medically appropriate and cost‑effective care.
Assist in maintaining a financially stable department and promote professional development with emphasis on evidence‑based practice and education supported by the Practice Governance Model.
Qualifications Required
Three years of experience in med‑surg or critical care nursing.
Bachelor's Degree (BSN).
Registered Nurse (KY).
Desired
Three years of experience in medical surgical or critical care nursing.
Accredited Case Manager or Certified Case Manager.
