For more than 55 years, Family Health Centers of San Diego (FHCSD) has dedicated its mission to providing caring, affordable, high‑quality health care and supportive services to all, especially to uninsured, low‑income, and medically underserved persons. As one of the country’s largest federally qualified health centers, FHCSD operates over 90 sites across San Diego County, including primary care, behavioral health, rehabilitation, dental, vision, substance use, mobile medical, urgent care, and pharmacy services. The organization serves more than 227,000 patients annually, 91% of whom are low‑income and 29% uninsured.

Responsibilities

Serve as a liaison between internal and external customers—including patients, other FHCSD staff, and external service providers—to improve patient/client quality of care and flow.

Complete all required documentation accurately, timely, and thoroughly, in accordance with department standards and grant/funder and insurance requirements.

Collaborate with the healthcare team on retention, appointment completion, and identification of barriers to care (e.g., quality improvement projects, case reviews).

Engage in patient/client interactions via telephone and in person to identify and meet needs, including facilitating referrals, resources, and follow‑up.

Assist with scheduling, organizing, and managing a patient/client’s healthcare maintenance and treatment.

Perform general clerical tasks, including filing, data entry, scanning, agenda creation, minutes, and encounter reports.

Use discretion in all interactions, answer routine and complex questions, and forward issues to the appropriate person while handling confidential information.

Perform other duties as assigned.

Education & Certifications

High School diploma (or GED equivalent) required.

2 years of experience in health education or a medical/health‑related field preferred.

Equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential duties.

Specific certifications may be required or preferred (e.g., CA HIV Test Counselor, limited CPT certification).

Must possess a valid California driver’s license, a car, and minimum required car insurance; an acceptable driving record is required.

Experience & Skills

Demonstrated ability to maintain good working relationships with employees, coworkers, and departments.

Intermediate interpersonal and customer service skills.

Strong written and verbal communication skills, including the ability to communicate with patients and families who speak English as a second language.

Demonstrated social skills, limit‑setting, and boundary maintenance.

Cultural sensitivity and respect for diversity.

Discretion with confidential information.

Team‑based and independent work ability.

Basic computer literacy (EMR documentation, background research, appointment follow‑up).

Basic supportive communication skills (reflecting, active listening, paraphrasing).

Basic organizational, time‑management, and detail‑orientation skills.

Knowledge of health and social service systems in San Diego County preferred.

Bilingual in English/Spanish preferred; may be required depending on assignment.

Strong computer applications and software skills.

Compensation and Benefits Salary range: $22.00 – $26.17 per hour.
