Pay Range: ASW/APCC/AMFT: $75,245.40 to $79,019.20 annually; LCSW/LPCC/LMFT: $88,088 to $107,660.80 annually

About CommuniCare+OLE Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high‑quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally‑qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school‑based programs.

Job Summary / Overview The Behavioral Health Clinician reports directly to the Behavioral Health Manager, Perinatal Mental Health and SUD. The position will provide in‑home/in‑community and in‑clinic engagement, service navigation, and individual and family counseling to Pregnant/Parenting clients enrolled in early intervention in‑home visiting and support programs. CommuniCare Health Center's mission emphasizes supportive, nonjudgmental care, preventive care, health education and community referrals to address the broad spectrum of health and psycho‑social concerns of clients served.

Minimum Position Requirements
Education: Master’s degree in social work/Marriage and Family Therapy/Mental Health and registered with the Board of Behavioral Sciences required.
Experience: Minimum one year working with clients with perinatal mood and anxiety disorders, involved in addiction treatment and/or involved with Child Welfare Services preferred.
Special Skills/Training:
Bilingual English/Spanish preferred.
Attendance and punctuality necessary.
Ability to work well with a multidisciplinary team.
Ability to problem‑solve and be self‑motivated.
Understanding substance use and best practices.
Commitment to excellence and high standards.
Excellent written and oral skills, strong organizational, problem‑solving, and analytical skills.

Valid Driver’s License required.
BLS Certification required.
Essential Functions and Responsibilities
Engages clients in appropriate services, utilizing effective rapport building and motivational interviewing skills.
Conducts screenings and intake assessments for clients utilizing appropriate assessment tools.
Works with clients to develop client‑centered treatment plans by establishing long‑ and short‑term goals and action steps to achieve goals.
Provides individual, group, couples, and family counseling and psychoeducation to clients in‑home, in‑clinic and in‑community through the Road to Resilience Grant.
Supports client success and stabilization in treatment services by providing case‑management, referral connection, monitoring progress and ongoing engagement efforts.
Completes all client information and documentation in a timely manner according to CCHC, First 5, and OCAP guidelines and standards, including required pre/post screening and data collection tools.
Collaborates with multidisciplinary team to identify best practices for services provided.
Responsible for detecting emotional, psychological, and physical danger signals with a non‑judgmental approach and offering feedback and support to clients.
Provides triage, crisis response, and support services to medical patients experiencing mental health symptoms, collaborating and communicating with medical staff regarding care coordination.
Responsible for communicating with and case‑coordination with referring agencies and other collaboratives including but not limited to First 5 Yolo County, Yolo County Children’s Alliance, Yolo Crisis Nursery, Yolo County Child Welfare Services, and Healthy Families Yolo County.
Attends collaborative multidisciplinary team meetings with internal and external providers.
Maintains ethical standards established in the field, including confidentiality.
Explains and enforces all rules and regulations associated with clinic treatment policies.
Compiles and reports statistics as required.
Attends clinical supervision and weekly staff meetings. Attends training/conferences as arranged.
Maintains standards listed in CommuniCare+OLE’s Personnel Policies and Procedures Manual and any other applicable standards.
Maintains current license/registration in good standing, submits all training certifications and CEUs to Human Resources, and attends ASAM Training once per year.
Completes other projects as assigned.
