Job Description
We are seeking an experienced Clinical Care Coordinator for our Wichita, Kansas office. The Coordinator will work closely with clinicians, staff, and patients to deliver high‑quality care services. Responsibilities
Develop individualized patient care plans and coordinate care using Ennoble Care’s electronic medical record system.
Maintain frequent contact with patients to provide care coordination, support, and ensure adherence to care programs.
Accurately document all client communications and care actions.
Communicate patient emergent needs to the primary clinician or designee and verify comprehensive care plans.
Inform the Team Supervisor of all care‑plan issues.
Work cross‑organizationally to meet patient, family, and partner facility needs.
Effectively use technology—including secure email, telephone, and electronic medical records.
Follow documentation protocols and internal communication best practices.
Demonstrate excellent customer‑service skills as reflected by positive feedback.
Support teamwork and contribute positively to the department.
Equipment Operation
Operate computer, telephone, copy machine, and other office equipment.
Use appropriate equipment to take vital signs.
Draw laboratory studies as ordered.
Qualifications
Comfortable communicating on the phone and in person for large portions of the day.
Compassionate and empathetic toward patients, demonstrating outstanding customer service.
Accurate note‑taking and documentation skills.
Strong multitasking ability across patients and workstreams.
Thrives in a fast‑paced environment.
Full‑time availability, Monday‑Friday, 8:30 am‑5:00 pm CST on‑site in Wichita, Kansas.
Proficiency with Microsoft Office applications (Outlook, Word, Excel).
Knowledge of basic healthcare terms and care principles.
Passes a drug screen and background check; positive attitude, adaptability, teamwork, and continuous learning.
Skilled at recording and interpreting vital signs.
Experienced in drawing laboratory studies as ordered.
Maintains strict hygiene and safety standards.
