Job Description Ennoble Care is looking for a full‑time, experienced Care Coordinator to work at our client’s senior‑living facility in Atlanta, Georgia, in the Marietta/Kennesaw metro area. This internal role aligns with our motto, “To Care Is An Honor.” The Care Coordinator will collaborate with clinicians, staff, and patients to reach healthcare goals and maintain open communication. The position requires compassion, experience, and strong organization skills. The coordinator will manage individual care providers, including scheduling and supporting caregivers and families.
Responsibilities
Complete individualized patient care plans and perform care management and coordination services using Ennoble Care’s electronic medical record system.
Frequent patient contact to provide care coordination, support, and manage compliance with care management programs to improve outcomes.
Document all client communications accurately.
Communicate with Primary Care Clinician or designee regarding patient emergent needs and life‑threatening episodes, ensuring comprehensive care plans are complete and accurate.
Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived issues.
Work with cross‑organizational areas to meet the needs of patients, families, and partner facilities.
Become skilled at using technology including secure email, telephone system, electronic medical records, etc.
Adhere to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communication.
Demonstrate excellent customer service skills with positive feedback from customers and patients.
Contribute as a positive member of the department, supporting all team members constructively.
Equipment Operation
Utilize a computer, telephone, copy machine, and other office equipment as necessary.
Qualifications
Comfortable speaking on the phone/in‑person for large amounts of the day.
Compassionate and empathetic toward patients/families, always demonstrating exceptional customer service.
Accurate and timely note‑taking for each task.
Ability to multitask between different patients and workstreams while remaining organized and efficient.
Thrives in a fast‑paced environment.
Full‑time availability Monday‑Friday daytime hours at the senior‑living facility.
Proficient in using a computer, including Outlook and Microsoft Office.
Knowledge of basic healthcare terms, conditions, roles, and care principles.
Must pass a drug screen, background check, maintain a positive attitude, adapt positively, be a team player, and be willing to learn continuously.
