Job Summary:

The Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient’s hospital stay and coordinates planning with other disciplines using a patient/customer driven approach.

Job Responsibilities

Perform comprehensive nursing assessment/reassessment.

Perform age‑appropriate admission or transfer assessment; obtain input from family or guardian when appropriate.

Accurately and completely document findings.

Assess post‑op/post‑invasive procedure patients.

Assess and document education and discharge needs of patient and family on admission and throughout hospitalization.

Provide patient reassessment and document pertinent observations per the care plan.

Establish, coordinate, and evaluate a plan of care based on assessment data, diagnosis, lab results, physician orders, protocols and standards of care.

Identify short‑ and long‑term goals based on patient care needs.

Formulate nursing interventions to achieve desired patient outcomes.

Incorporate evidence‑based practice into the nursing care plan.

Provide and document nursing interventions based on the assessment and plan of care.

Collaborate with health team members for daily coordination of care.

Communicate patient care details, including accurate hand‑off reports.

Administer and document medications accurately per hospital policies.

Monitor and document IV fluids and parenteral nutrition per policies.

Complete referrals and request consultations as indicated.

Provide patient/family education and discharge planning per guidelines.

Clarify and implement physician orders accurately and promptly.

Assist physicians with procedures or delegate to care partner as appropriate.

Document “read‑back” for all telephone orders; take telephone orders only in emergencies.

Recognize changes in the patient’s condition and respond accordingly.

Involve family or guardian in care and decision‑making as appropriate.

Identify risks and take appropriate actions.

Incorporate infection control practices in daily care.

Document and communicate nursing care and any changes in patient condition.

Evaluate ongoing effectiveness of care and modify plans as needed.

Monitor patient response to medications, treatments, and procedures.

Evaluate the effectiveness of patient/family education.

Report variations from care and follow incident reporting policies.

Perform specialized care for patients at high risk, including restraint care, pain management, and abuse assessment.

Adhere to all quality and performance standards; participate in learning and quality improvement activities.

Maintain currency in clinical knowledge, policies, and procedures.

Participate in staff meetings, committees, and shared governance activities.

Support and mentor new employees and students.

Implement safe patient handling techniques and ensure a clutter‑free environment.

Maintain personal professional appearance and adhere to dress code.

Complete mandatory requirements: licensure, BLS, HI, TB screening, influenza vaccination, ethics, safety, and information security trainings.

Provide excellent patient service, communicate effectively, and maintain confidentiality.

Assist in developing unit‑based performance improvement plans and lead improvement teams as needed.

Offer resource stewardship, cost‑effective services, and documentation of chargeable supplies.

Document and review nursing quality indicators and participate in performance improvement activities.

Provide age‑appropriate care for adult and geriatric patients, and ensure cultural competence in pain management.
