In this role, you will be responsible for supporting the overall health and well‑being of residents by delivering high‑quality, person‑directed care and nursing services within the Primary Care Office. This role is essential in advancing the Acts Signature Well‑Being Model by contributing to interdisciplinary care, supporting proactive health monitoring, and facilitating access to well‑being resources. You will collaborate closely with the Nurse Practitioner and the broader wellness team to foster a holistic, resident‑centered approach that promotes independence, dignity, and thriving in place.

Requirements

Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing

Minimum of two years of clinical nursing experience, preferably in a senior living, primary care, or wellness‑focused environment

Prior experience in office management, care coordination, or leadership roles is strongly preferred

Current certification in CPR, AED, and first aid (or obtain within three months of employment)

Pay Range $24.72 - $31.50 / hour. Starting rate will vary based on skills and experience.
