Third Eye Health is seeking experienced, multi-state licensed APRNs to join our telehealth team. This role involves providing weekend daytime and overnight coverage for skilled nursing facilities (SNFs) nationwide. Ideal candidates will have:
A minimum of 5 years of clinical experience, including 3 years as an Advanced Practice Provider in a post-acute, SNF, or long-term care setting. Active multi-state licensure and willingness to obtain additional state licenses as needed. Ability to work Saturday and Sunday between 11am -11pm CT.

Telehealth Clinician - NP
Telehealth Clinician’s (Clinicians) manage post-acute patients in a SNF through the Third Eye Health telemedicine application with the goal of treating in place. Telehealth Clinicians are hired in W2 employment or independent contractor statuses, paid hourly with malpractice coverage included. Desired coverage includes daytime, evening, weekend and holiday hours. The ideal Telehealth Clinician is multi-state licensed or willing to pursue multi-state licensure.
Job Summary: The Clinician conducts comprehensive assessments of patients via telehealth. The assessments include medical and social history, and a visual, limited physical examination. Utilizing strong clinical decision-making skills which display a solid knowledge of disease pathophysiology and pharmacology, the clinician will effectively manage acute conditions of post-acute patients including new admissions stabilizations. The scope of this position is treating and prescribing. All encounters are documented in the provider EHR.
Duties/ Responsibilities: · Conducts comprehensive assessments of patients via telehealth. The assessments include a review of medical and social history, and a limited visual physical examination. · Utilizes an extensive professional level of medical knowledge including pharmacology, pathophysiology, clinical assessment skills in the adult and geriatric population and chronic disease management to assess, diagnose and prescribe treatment of patients. · Accurate and timely documentation of all patient encounters are required within two (2) hours of the end of a shift. · Meet and complete all CME for state licensure and maintain state licenses when requested by the Company. · Participation in educational programs that increase clinical knowledge base and skill set and demonstrate accountability for professional practice. · Requires an individual to maintain the ability to work in an environment with PHI / PII data. · Weekend and/or evening work is required.
Required Skills/ Abilities: · Knowledge of customer service practices and principles · Excellent listening, verbal, and written communication skills · Ability to work independently and be self-motivated to meet deadlines and achieve goals. · High level of ethics, integrity, discretion, and confidentiality. · Strong computer skills: Microsoft office preferred. · Strong attention to detail.
Education and Experience: · NP · Board certified · Internal, Family or Emergency Medicine · Five (5) or more years of experience in patient care or clinical work post-residency training.
Physical Requirements: · An ability to perform the essential job functions consistently and successfully with the ADA, FMLA and other federal, state, and local standards, including meeting productivity standards. · Prolonged periods of sitting at a desk and working on a computer, tablet or mobile device. · Must be able to lift up to 15 pounds at times. · Occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop; kneel, crouch or crawl; talk or hear. · Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
Working Conditions: · Work will be done in the home office setting. · Must maintain a HIPAA compliant dedicated space in the home office setting to complete documentation and to conduct telehealth visits. This space must provide privacy to protect PHI/PII and provide a professional backdrop for video conferencing. · Wi-Fi required.
