Job Summary Provide care, coordinate, and support Phases I, II and III for Cardiac and Pulmonary Rehabilitation, and Supervised Exercise Therapy for Peripheral Artery Disease patients. Collaborate with a team of Exercise Physiologist, Cardiac Rehabilitation Nurse, and Registered Respiratory Therapist to implement and design exercise programs tailored to the needs of patients recovering from Cardiac, Pulmonary and Vascular conditions. The expertise will help improve the patient’s cardiovascular functions, manage symptoms, educate about risk factor modification, and improve quality of life.

Responsibilities
Referral/Enrollment/Registration/Scheduling: Demonstrates ability to competently perform complete referral, scheduling, and registration workflow processes for cardiac and pulmonary rehabilitation program services.
Exercise Prescriptions: Collaborates with cardiopulmonary rehabilitation team members to develop a safe, effective exercise prescription (frequency, intensity, time, type) based on initial assessment, medical history, test results, physician orders, limitations, and individualized goals.
Walk Studies and Diagnostic Testing: Performs complete and accurate 6‑ and 12‑minute walk studies and diagnostic testing, including exercise‑induced asthma studies and metabolic studies.
Individual Treatment Plan (ITP): Works collaboratively with team members, medical director, and patient in developing and documenting the ITP, considering physician’s orders, medical history, assessments (Functional, Nutritional, Psychosocial), limitations and patient goals.
Direct Patient Care: Assists patients in executing their ITP/exercise prescription by instructing, encouraging, monitoring, and assisting them.
Patient Intakes/Orientations: Supports comprehensive initial intake and assessment (Functional, Nutritional, Psychosocial) to gather relevant patient needs with patients, family members, and team.
Patient Education, Counseling, and Coaching: Identifies risk factors, develops an educational program, and utilizes interdisciplinary approach.
Assist in AACVPR certification and recertification in Cardiac and Pulmonary Rehabilitation.
Complete competencies and mock codes per AACVPR requirements.
Represent the department with educational outreach to community and other services.
Mentor, orient, and assist in training staff and interns.
Safety and Emergency Procedure: Perform patient care with emphasis on safety, initiate and assist with emergency care per policy and procedures.
Outcome and Performance Improvement: Assess outcome and performance improvement measures in rehabilitation programs.
All other duties as assigned within scope and range of job responsibilities.
Required Education, Credentials, and Experience
Education: Bachelor’s Degree in Exercise Physiology, Exercise Science, Kinesiology, or related field with emphasis in Cardiac or Pulmonary Rehabilitation.
Credentials: Basic Life Support.
Experience: One year in Cardiac/Pulmonary Rehabilitation and/or skilled internship in Cardiac Rehabilitation.
Preferred Qualifications
Education: Master’s Degree in Exercise Physiology.
Credentials: Medical Exercise Specialist, Advanced Cardiovascular Life Support (ACLS), ACSM Exercise Specialist certification, ACE Exercise Specialist certification, CCRP certification from AACVPR.
Experience: Five years’ experience in Cardiac and Pulmonary Rehabilitation.
