Summary Serves as a patient advocate, liaison, and adviser/educator to assist patients with navigating the continuum of care.
Responsibilities
Serves as a liaison between patient and clinical staff, administration, physicians, and community/external resources to coordinate and maximize resources and identify and resolve barriers to the plan of care.
Provides emotional support, counseling, clinical education and expert guidance to patients and families to promote understanding and meaningful participation in the healthcare process and personal decision‑making.
Assists patients and families with resolving financial, psychosocial, functional, and administrative issues by advising options and referring to appropriate resources; intervenes as appropriate to advocate for patient and family.
Continuously reviews and evaluates patient progress as outlined within the plan of care; communicates plan of care to patient and family and solicits concerns, questions, and issues for resolution.
May collect and assess data to identify quality trends or actual sources of risks to patients and employees; collaborates with multidisciplinary team and committees with respect to quality outcomes.

Additional Job Information Complexity of Work: Requires critical thinking skills, effective communications skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Required Work Experience: One (1) year of experience in a related field.
Other Information: If RN, must be licensed. Master’s in Social Work (MSW) degree preferred if degreed. (RT) and certified in Radiation Therapy (T) by the American Registry of Radiologic Technologists (ARRT) – Radiation Therapy license required if working in Radiation Oncology.
