Care Manager, Critical Time Intervention (CTI)
ACMH, INC. is recruiting for the Care Manager, Critical Time Intervention (CTI) role in Manhattan. The position serves as a member of the CTI Team, providing time‑limited intensive care coordination for a caseload of high‑risk clients. Responsibilities include developing person‑centered care plans, providing linkage to resources, and engaging individuals in skill building.
Responsibilities
Engage clients at hospital bedside to provide program overview and conduct intake assessments.
Develop rapport with clients to engage them in improving their health and wellness.
Participate in weekly team meetings to discuss new referrals and provide updates on enrolled clients.
Administer standardized health and psychosocial risk screenings according to CTI protocols and timeframes.
Use screenings to identify client needs and barriers to aid in the development of a comprehensive care plan.
Work in collaboration with the client and members of the inpatient and outpatient care team to identify and prioritize client’s healthcare and psychosocial goals.
Manage the client’s care plan overall, coordinating all aspects of care and supporting adherence to care plan goals, including medications and other treatments; and document care plan progress toward goals.
Identify, facilitate and secure access to needed healthcare, social services benefits, and community resources. Collaborate with other care team members and providers, including behavioral health, disease care management, home care, social work and community‑based organizations, to help the client achieve optimal health outcomes.
Provide comprehensive transitional care following hospitalization events in accordance with Critical Time Intervention (CTI) protocols.
Facilitate care delivery by scheduling appointments, obtaining necessary information, and arranging transportation.
Embrace the team model by collaborating with members of the team and providing support as needed.
Use evidence‑based practices, such as motivational interviewing, to empower clients to grow and attain goals.
Work with family members and other collaterals of the client’s choice to facilitate planning or delivery of care.
Implement tasks outlined on the care plan and ensure follow‑up and continuity of care between client interactions.
Document all interventions and attempted contacts in the EHR in accordance with program standards.
Assess domiciled client’s living conditions by conducting home visits.
Attend in‑service training as requested.
Perform duties as assigned by supervisor.
Qualifications B.A. or M.A. degree in social services or a related field and at least one year of experience providing direct service to individuals who have mental health disorders in the human service field, nursing or care management/service coordination. Demonstrated competency in clinical documentation, stage‑wise treatment and motivational interviewing. Strong written and verbal communication skills.
Compensation $61,560 plus generous benefits.
