Responsibilities
Lead and oversee the implementation of the Critical Time Intervention (CTI) model to ensure program fidelity and effectiveness.
Supervise and provide guidance to CTI team members, including case managers, an LPN/RN, and Peer Specialists.
Develop and maintain collaborative relationships with community partners, service providers, and stakeholders to facilitate client support networks.
Conduct regular team meetings to provide guidance and information to all team members.
Ensure that all client documentation and records are accurate, up-to-date, and compliant with agency and regulatory standards.
Monitor and evaluate program outcomes to identify areas for improvement and develop action plans to address them.
Facilitate professional development and training opportunities for team members to enhance their skills and knowledge.
Report program performance and outcomes to leadership, ensuring transparency and accountability.
Advocate for clients’ needs and rights within the organization and in the broader community.
Qualifications
Valid driver’s license and reliable transportation.
Flexibility to work occasional evenings or weekends as needed.
Clinical License in Social Work (LMSW/LCSW), Mental Health Counseling (LMHC), or a related field.
Minimum of 3 years in a supervisory role.
Knowledge of Microsoft Suite Applications.
Salary starting at $68,000 - $71,000/year depending on education, credential and experience.
