Position Summary: This role supports the Director of Nursing Services in the day‑to‑day management of nursing activities within a long‑term care facility. It involves the development of policies, coordination of personnel, and execution of resident care services.

Administrative Functions Assist the Director of Nursing Services in directing the day‑to‑day functions of nursing activities in accordance with current rules, regulations, and guidelines governing the long‑term care facility.

Participate in developing, maintaining, and updating written policies and procedures that govern the day‑to‑day functions of the nursing service department.

Ensure reference material (PDRs, regulations, professional standards of practice, etc.) at the nurses' stations is current; recommend written material that will assist the nursing service department in meeting day‑to‑day needs.

Ensure Nursing Service Procedures Manual is current and reflects the day‑to‑day nursing procedures performed in this facility; all nursing service personnel must comply with procedures set forth in the Manual.

Make written and oral reports/recommendations to the Director regarding the operation of the nursing service department; review policies, procedure manuals, job descriptions, etc., periodically and make recommendations for revisions.

Assist in developing methods for coordinating nursing services with other resident services to ensure continuity of the residents' total care plan; ensure all nursing service personnel are in compliance with their respective job descriptions.

Participate in the development, maintenance, and implementation of the facility's quality assurance program for the nursing service department, including facility surveys (inspections) by authorized government agencies.

Periodically review the resident's written discharge plan; participate in the updating of the resident's written discharge plan as required; assist the Director in planning the nursing services portion of the resident's discharge plan.

Meet with nursing staff and support personnel in planning the department's services, programs, and activities; admit, transfer, and discharge residents as required.

Complete accident/incident reports, reports of meeting house policy, and complete medical forms, reports, evaluations, and charting as necessary.

Agree not to disclose assigned user ID code, password for accessing resident/facility information, and report suspected violations to the Administrator; report any known or suspected unauthorized attempts to access the facility's information system.

Committee Functions Attend various committee meetings of the facility (Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment, etc.) as required. Assist in preparing written and/or oral reports of nursing service programs to submit to such committees as directed.

Provide information to the Quality Assurance and Assessment Committee as requested; participate in developing the agenda for Care Plan Committee meetings and reviews regularly scheduled discharge plans.

Implement recommendations from established committees as instructed by the Director.

Personnel Functions Ensure that a sufficient number of licensed practical and/or registered nurses are available for your tour of duty; ensure a sufficient number of certified nursing assistants are available to meet daily nursing care needs.

Develop work assignments; delegate to each Charge Nurse the administrative authority, responsibility, and accountability necessary to perform his/her assigned duties.

Ensure all nursing assistants are enrolled in or have graduated from an approved nursing assistant training program; ensure all nurse aide trainees are supervised by a licensed nurse.

Make daily rounds of the nursing service department; report findings to the Director; monitor absenteeism to ensure adequate duty coverage.

Report problem areas to the Director; ensure dress code and other policies are followed; review complaints and grievances filed by personnel; make appropriate reports to the Director.

Develop and maintain a good working rapport with inter‑department personnel; create an atmosphere of warmth, personal interest, and positive emphasis throughout the unit and shift.

Meet with shift regularly to identify and correct problem areas; review and evaluate department workforce and make recommendations to the Director.

Participate in employee performance evaluations, staffing requirements, and recommendations concerning employee dismissals, transfers, etc.; administer departmental disciplinary action fairly and consistently.

Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policies; report known or suspected incidents of fraud to the Administrator.

Ensure computer workstations left unattended are properly logged off or the password‑protected automatic screen saver activates per policy.

Nursing Care Functions Inform nursing staff of new admissions, expected time of arrival, room assignment, etc.; ensure rooms are ready for new admissions; greet newly admitted residents and escort them to rooms as necessary.

Make rounds with physicians as necessary; encourage physicians to record and sign progress notes and orders promptly.

Consult with residents, families, and physicians regarding resident care plans; notify attending physicians and next of kin of changes in resident condition.

Visit residents daily to observe and evaluate physical and emotional status; ensure direct nursing care is provided by a licensed nurse, certified nursing assistant, or nurse aide trainee as appropriate.

Review nurses' notes for completeness and descriptiveness; provide direct nursing care as necessary (IVs, sputum/urine/other specimens, TPRs, blood pressures, etc.).

Admit, transfer, and discharge residents as necessary; assist in developing and implementing corrective action; keep Director informed of status and related matters via written/oral reports.

Assist Charge Nurse in monitoring seriously ill residents; authorize restraints in accordance with facility policies.

Ensure RN and LPN comply with medication procedures, including storage, administration, and control; monitor medication passes and treatment schedules.

Review medication cards for completeness, accuracy of orders, and adherence to stop‑order policies; inspect treatment areas daily for cleanliness and safety.

Ensure residents unable to call for help are checked frequently; report problems to Director; aid in transporting discharged residents; inform families of resident death and request funeral services when applicable.

Staff Development Plan, conduct, and schedule in‑service training classes on nursing practice; assist In‑service Director/Educator in developing annual program (OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control, etc.).

Assist Director in planning clinical supervision for nurse aide trainees; attend and participate in continuing education programs to maintain licensure status.

Safety and Sanitation Assist Safety Officer in developing safety standards; assist Director and/or Infection Control Coordinator in evaluating routine and job‑related functions for blood/body fluid exposure.

Ensure all personnel performing tasks with potential exposure receive appropriate training; ensure adequate supply of personal protective equipment and enforce disposal procedures.

Monitor shift personnel for compliance with safety regulations; enforce hand hygiene, fire safety drills, and disaster preparedness.

Participate in infection control program implementation and maintenance; report hazardous conditions or equipment failures; enforce use of safety equipment during lifting/moving residents.

Equipment and Supply Functions Recommend equipment and supply needs to Director; maintain adequate stock of medications, supplies, and equipment.

Assist Maintenance Director with preventive maintenance program; ensure only trained and authorized personnel operate equipment.

Monitor nursing procedures to ensure efficient use of supplies; maintain MSDS documentation for hazardous chemicals.

Care Plan and Assessment Functions Develop written preliminary and comprehensive assessments of each resident's nursing needs; help create written plans of care outlining problems, goals, actions, and responsible services.

Encourage resident participation in care plan review; ensure personnel access and follow care plans; review nurses' notes for plan compliance.

Assist Resident Assessment/Care Plan Coordinator in scheduling and revising MDS, RAPs, and Triggers.

Budget and Planning Functions Monitor economic conditions; recommend adjustments in nursing services to ensure continued high‑quality care; report fraud incidents or false billings.

Supervisory Requirements Overall supervision and management of nursing staff; delegate administrative authority as required for assigned duties.

Qualifications – Education & Experience Minimum: Nursing Degree from an accredited college or university.

Training in rehabilitative and restorative nursing practices required.

Two years or more experience preferably in a long‑term care facility.

Language, Technical & Mathematical Skills Read, write, speak, and understand English; read technical procedures, policy and procedure manuals; present information and respond to questions.

Apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning & Leadership Make independent decisions and solve practical problems; deal tactfully with personnel, residents, families, visitors, government personnel, and general public.

Leadership and supervisory ability with willingness to work harmoniously with and supervise other personnel.

Professional Knowledge & Conduct Provide care appropriate to age‑related needs; knowledgeable of nursing, medical practices, laws, regulations, and guidelines.

Display patience, tact, cheerfulness, enthusiasm, and willingness to handle difficult residents.

Must not pose direct threat to health or safety of others; maintain confidentiality per HIPAA.

Certificates, Licenses, Registrations Current, unencumbered, active RN license in state; CPR certification; knowledge of PCC preferred.

Maintain required continuing education and licensure; remain in good standing with State Board of Nursing.

Physical Demands Move intermittently throughout the workday; speak and write in English; cope with mental and emotional stress; see and hear or use prosthetics if necessary.

Normal health conditions required; lift/push/pull ≥25 pounds up to 5 feet and carry same weight 50 feet; assist in evacuation during emergencies.

Work Environment Office areas and throughout nursing service area (drug rooms, nurses' stations, resident rooms); frequent interruptions; exposure to residents, personnel, visitors, agencies; hostile and emotionally upset environment; possible call‑back during emergencies.

May involve injury from falls, burns, odors, chemicals, infectious waste, TB, HIV, COVID, hazardous chemicals, etc.; must comply with safety regulations.
