Inpatient Case Manager – Part Time (20hrs) Weekend Havenwyck Hospital is seeking a dynamic and talented licensed Social Worker to join our compassionate clinical team. The Social Worker provides social services to all inpatient and day‑hospital patients.

Shift Options

7:00 a.m. – 5:30 p.m.

7:30 a.m. – 6:00 p.m.

8:00 a.m. – 5:30 p.m.

Responsibilities

Participate as an integral professional member of a multidisciplinary treatment team led by the psychiatrist.

Facilitate the therapeutic milieu on the treatment unit.

Perform social work interventions as directed by the treatment team, treating psychiatrist, and/or Lead Social Worker.

Obtain and assess social history information and report findings to treatment teams.

Execute discharge planning and case management from the day of admission.

Provide group, individual, family, substance‑abuse, and/or couples therapy as appropriate.

Act as liaison for hospitalized patients with community agencies and resources.

Attend treatment team conferences and complete treatment plans.

Document all activities in the medical record.

Qualifications

Master of Social Work degree from an accredited school or equivalent MA.

Current Michigan license as a Licensed Master Social Worker (LMSW), LLMSW, Licensed Professional Counselor (LPC), or Limited Licensed Counselor (LLC).

Experience on an inpatient psychiatric unit preferred.

Employee must be in good standing to be considered for an internal transfer.
