Job Description The Post-Acute Coordinator is responsible for overseeing, executing, and verifying post-acute transition plans for patients. This role will address requests for post-acute services from both inpatient clinicians and community-based providers.

Qualifications
High school diploma or equivalent is required.
2 yrs of an equivalent combination of education and experience in a hospital/medical related field is required.
Additional Information Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella. MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.
