Transition Care Program – Commencing 2026 (Casual Roles)
We are seeking qualified Personal Care Workers (PCWs) to join our new Transition Care Program, commencing 2026, on a casual employment basis.
About the Transition Care Program
The Transitional Care Program provides goal-oriented, time-limited, and focused care to support older people following a hospital stay. The program aims to help clients regain independence and return home, rather than prematurely entering residential aged care.
Services include:
Nursing support
All services are designed to maintain and improve physical and cognitive functioning.
The program supports:
Adults aged 65 years and over
Aboriginal and Torres Strait Islander clients aged 50 years and over
Clients with an age-related illness
For Personal Care Workers:
Certificate III in Aged Care, Disability, or Individual Support
For Enrolled Nurses:
Current AHPRA registration as an Enrolled Nurse
For All Applicants:
NDIS Worker Screening Check (NDIS Workers Screen Card)
NDIS Orientation Module Certificate
Current First Aid & CPR
National Criminal Record Check (Aged Care)
Current mandatory vaccinations apply (NSW Health -Covid -19, Hepatitis B, Influenza)
Current driver’s licence with comprehensive insurance
Reliable vehicle
Ability to work within a multidisciplinary, client-centred care model
