We are seeking qualified Personal Care Workers (PCWs) to join our new Transition Care Program, commencing 2026, on a casual employment basis.
About the Transition Care Program
The Transitional Care Program provides goal‑oriented, time‑limited, and focused care to support older people following a hospital stay. The program aims to help clients regain independence and return home, rather than prematurely entering residential aged care.
Services include:
Nursing support All services are designed to maintain and improve physical and cognitive functioning.
The program supports:
Adults aged 65 years and over Aboriginal and Torres Strait Islander clients aged 50 years and over Clients with an age‑related illness
For Personal Care Workers:
Certificate III in Aged Care, Disability, or Individual Support
For Enrolled Nurses:
Current AHPRA registration as an Enrolled Nurse
For All Applicants:
NDIS Worker Screening Check (NDIS Workers Screen Card) NDIS Orientation Module Certificate Current First Aid & CPR National Criminal Record Check (Aged Care) Current mandatory vaccinations apply (NSW Health -Covid -19, Hepatitis B, Influenza) Current driver’s licence with comprehensive insurance Reliable vehicle Ability to work within a multidisciplinary, client‑centred care model
