Job Responsibilities

Under general supervision of the Tuberculosis (TB) Unit Manager, provides social work services for persons receiving care for TB. Works with the TB Nurse Case Manager to promote completion of recommended treatment to include providing medication delivery for treatment of TB infection and/or disease, assists patient with initiation of Video Directly Observed Therapy (VDOT). Provides counseling, emotional/mental health support and linkage to care to clients dealing with various health issues, including chronic illness, mental health disorders, and substance use concerns. Provides psychosocial assessment, treatment and follow‑up services to a diverse population of clients and their contacts to include substance abuse and mental health needs. Provides short‑term therapy to assist individuals/families adjust to health‑related problems, assists clients in developing long‑range plans to address health problems, makes appropriate referrals to community resources and develops discharge plans when social work services end. Goals and treatment plans are set in collaboration with the individual/family. Documentation will meet policy standards and patient confidentiality will be maintained. Provides patient education, initiates referrals and counsels individuals, caregivers and families in the clinic, household and community settings. Assists with contact investigations as needed. Facilitates patient access to community resources and assists in discovering resources for persons with active and suspected TB or Latent Tuberculosis Infection (LTBI); participates in the development of social work program policies as indicated. Provides medication delivery for treatment of TB infection and/or disease, assists patient with initiation of VDOT, and notifies Nurse Case Manager immediately of any missed appointments. Utilizes incentives and enablers to assist with adherence. Provides groceries and transportation with TB Unit Manager approval. Participates in Continuous Quality Improvement (CQI) activities such as record reviews, program evaluations, team meetings, etc. Maintains records and reports in accordance with agency standards including documentation in the Electronic Health Record (EHR), Personnel Cost Accounting System (PCAS), weekly reports, etc. Conducts Social Work (SW) peer audits as designated. Assists with agency shelter operations, and client emergency response including First Aid, Cardiopulmonary Resuscitation (CPR), and Automated External Defibrillator (AED), and be willing and available to assist during times of public health emergency and outbreak response efforts. Other duties as assigned. Minimum and Additional Requirements

State Minimum Requirements: A bachelor's degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board. Agency Additional Requirements: Must have a Master of Social Work (MSW) from a school accredited by the Council of SW Education and be licensed as a Licensed Master Social Worker (LMSW) or Licensed Independent Social Worker (LISW) by the SC Board of Social Work. Must have a valid driver's license. May be required to use personal vehicle. Must be able to make home visits. May need to use stairs to enter client homes and/or encounter domestic animals. Significant travel and/or overnight stay within the state may be required. May require flexible work hours. Must be able to pass a fit test and be able to use appropriate respiratory protective equipment. Employee will be in contact with infectious or potentially infectious tuberculosis (TB) cases/suspects. Must obtain Basic Life Support (BLS)/CPR/First Aid certification within thirty (30) days of hire or provide documentation of satisfactory course completion prior to employment. Must participate in disaster preparedness during hazardous weather or declaration of an emergency; subject to duty and/or call on a 24‑hour basis. Preferred Qualifications
