Overview Description

Primary responsibility is for the clinical case management of the transplant recipient involving all phases of the transplant process, from initial referral through outpatient follow-up. This position requires an advanced level of expertise and overall understanding of organ transplantation for the organ type specified. Responsibility involves coordination of interdisciplinary medical care delivery system under the direction of the Medical Director; liaison functions with patient, family, medical staff, referring physicians, and insurance companies; individualized patient/family teaching; assessment of appropriate medical intervention for effective use of resources and quality assurance. Additional responsibilities include data collection for clinical management and certification requirements, development of educational materials and programs for patients, nursing staff and physicians. Develop clinical protocols, and policy and procedures. Community education and public relations. Transplant donor/recipient coordination and liaison function with organ procurement agencies.

Responsibilities

Coordinate the clinical case management of transplant recipients across all phases from referral to post-transplant follow-up, requiring advanced knowledge of organ transplantation.

Coordinate interdisciplinary medical care under the direction of the Medical Director; liaise with patients, families, medical staff, referring physicians, and insurance companies; provide individualized patient/family teaching; assess medical interventions for efficient resource use and quality assurance.

Collect data for clinical management and certification requirements; develop educational materials and programs for patients, nursing staff, and physicians.

Develop clinical protocols, policies, and procedures.

Engage in community education and public relations; coordinate donor/recipient activities with organ procurement agencies.

Report to the Nurse Manager; receive supervision from the Transplant Program Medical Directors and the Nurse Manager; participate in performance evaluation with these leaders.

Compensation

TC1: $171,905-$231,099 (annually)

TC2: $207,129-$247,553 (annually)

Qualifications

TC I: Current California Registered Nurse license, graduation from an accredited nursing program with a B.S. degree or Master\'s and 5 years of relevant RN experience with a combination of critical care and/or transplant experience (required).

TC II: Current California Registered Nurse license, graduation from an accredited nursing program with a B.S. degree or Master\'s, CCTC and a minimum of 5 years relevant RN experience with a combination of critical care and/or transplant experience. Must have CCTC or CPTC (required).

Ability to case manage all aspects of a transplant patient\'s medical care from initial evaluation to post-transplant follow-up; broad knowledge of organ transplantation including immunology, immunosuppression, rejection and complications, drug toxicity and graft failure (required).

Ability to communicate effectively with people from diverse cultural backgrounds; strong verbal and written communication skills (required).

Ability to interface effectively with patients and their families (required).

Excellent organizational skills, ability to meet deadlines, work independently, prioritize workload, and manage numerous concurrent tasks in a high-stress environment (required).

Ability to coordinate and assess patient care and act as a liaison with nursing staff, outside agencies, team physicians, referring physicians, patients and families (required).

Ability to teach about transplantation and medical management issues (required).

Pediatric experience (required).
