About the Role Do you have a heart for seniors and a passion for supporting their emotional and social well-being? We are seeking a Social Services Assistant to join our compassionate team and help ensure that each resident receives the support they need to thrive in our care community. As a Social Services Assistant, you'll play a key role in coordinating and providing psychosocial support to residents and their families, assisting with admissions, discharges, and transitions, and advocating for the dignity and well-being of those we serve.
Key Responsibilities
Assist with admissions and discharges by coordinating assessments, documentation, and family communications.
Provide emotional support and crisis intervention for residents and families.
Facilitate care planning and interdisciplinary meetings.
Advocate for residents’ rights and personal preferences.
Help identify and address resident needs, including mental health, behavioral, or social issues.
Document assessments, progress notes, and social histories accurately.
Coordinate referrals to community resources and support services.
Participate in developing and implementing individualized care plans.
Qualifications
High school diploma or equivalent required; associate or bachelor’s degree in social work, Psychology, or Human Services preferred.
Prior experience in a long‑term care or healthcare setting strongly preferred.
Knowledge of state and federal regulations for nursing homes a plus.
Empathy, patience, and excellent communication skills.
Ability to maintain confidentiality and professional boundaries.
