Aviata Health Group is looking for a qualified Social Services Assistant to join our resident‑ and family‑focused community. The primary purpose of this position is to ensure the medically related, emotional, and social needs of residents are met on an individual basis.

Major Responsibilities

Maintain confidentiality of resident health information and report violations to the Executive Director/LNHA

Meet with residents and families during admissions to review policies on advance directives, resident rights, and non‑discrimination

Conduct and document social services evaluations, identifying resident problems and needs

Arrange for social work services as indicated by resident/family needs

Assist residents, as needed, with financial planning

Keep families informed of resident problems, personal needs, transfers, and changes of level assignment

Collaborate with facility staff regarding resident issues

Ensure compliance with all corporate, state, federal, and regulatory standards

Assist with discharge planning

Develop and implement innovative ideas and concepts that improve systems and processes for superior results

Minimum Qualifications

High school diploma or equivalent

One (1) year of experience in long‑term care preferred

Demonstrated ability to assess resident needs and coordinate the discharge planning process

Qualified, compassionate, and dedicated to doing a good job
