What You Will Do
Assess the needs of disabled children and their families and develop tailored care and support plans
Provide guidance and ongoing support to children and families, helping them access appropriate services and resources
Work collaboratively with multidisciplinary teams to deliver coordinated and effective care
Ensure compliance with statutory duties, policies, and best practice guidelines
Advocate for the rights, wellbeing, and equality of children and families
