Overview
The Fargo VA Health Care System is recruiting for 1 full-time Social Worker for Primary Care Outpatient Clinics at the Grand Forks VA Clinic, 3221 32nd Ave S, Grand Forks, ND 58201.
Responsibilities
Provide Veterans in the Primary Care Outpatient Clinics with high quality case management, clinical treatment, advocacy, and coordination with VA and community service providers / agencies as needed by the client, in collaboration with the primary care interdisciplinary treatment teams.
Demonstrate innovation, creativity, expertise, and skill in an expeditious and efficient manner; provide cross-coverage as needed.
Psychosocial Assessment
— develop an assessment of the veteran, family, economic, community, and psychosocial support systems when clinically indicated; reflect a high level of skill in gathering, analyzing, interpreting, and documenting data relevant to patient health care planning, treatment, education, and case management.
Clinical Diagnosing using DSM-5 and ICD-10 behavior codes
— demonstrate knowledge in DSM-5 diagnosing and document diagnostic impressions in psychosocial assessments when appropriate; may function independently or within a team to determine diagnosis and formulate a treatment plan.
Treatment Planning/Goal Setting
— demonstrate knowledge of psychosocial treatment modalities and the ability to implement treatment modalities with individuals, families, and groups to achieve treatment goals; develop a treatment plan and set achievable goals with the veteran/family in collaboration with the primary care interdisciplinary team when indicated.
Therapy
— provide ongoing psychotherapy (group and/or individual) and/or supportive counseling; utilize DSM-V to ensure appropriate treatment recommendations.
Referral to service providers
— act as the subject matter expert on VA and/or community resources; collaborate with other providers to reassess needs and establish referrals based on veteran preference or surrogate decision-maker.
Resource Development
— develop a resource file of VA and community social service programs and refer the veteran to needed services.
Advocacy
— advocate for the veteran/family when appropriate; educate and encourage self-advocacy to foster independence and empowerment.
Crisis Intervention
— perform rapid assessments and develop crisis management plans to maintain the patient at home or facilitate admission to appropriate placements.
Work Schedule Monday - Friday, 7:30 a.m. – 4:00 p.m. or 8:00 a.m. – 4:30 p.m.

Child Care Subsidy: After 60 days of employment, eligible for subsidy up to 25% of eligible childcare costs (max $416.66 monthly) for eligible children
