The Case Management Assistant collaborates with and assists the case management team(s) with the provision of case management administrative services in an effort to facilitate enrollment, maintain regulatory compliance, and achieve optimal outcomes.
Education Bachelors in Social Work, Sociology, Psychology, Nursing or a related field - Preferred
Experience 1 Year experience in the human service or health related field- Preferred
License & Certification Professional License in any clinical practice a plus.
Core Job Functions
Assists with managing referrals and facilitating the enrollment process initially and during recertification.
Assists with patient discharges and transfers, both internally and externally.
Assists with managing patient authorizations.
Completes assigned reports timely and accurately and orders patient supplies.
Assists manager with preparing for and conducting department meetings, conferences, and training sessions.
