Clinical Mental Health Provider - Youth ACT Team

Job Description Job Description Annual Salary: $65k - $75k (varies depending on years of experience) The Youth Assertive Community Treatment (ACT) Program delivers comprehensive, community-based behavioral health care for children and adolescents ages 10-21 who experience serious emotional disturbance or complex mental health challenges. The program’s mission is to strengthen family functioning, improve stability, and support successful reintegration for youth transitioning from inpatient or residential care. Services are youth-guided, family-centered, and trauma-informed, delivered by a multidisciplinary team focused on promoting healing and independence across home, school, and community settings. Position Summary The Licensed Behavioral Health Clinician plays an integral role on the ACT team, providing direct clinical interventions, counseling, and family support in home and community environments. This position emphasizes individualized treatment, skills-building, and family empowerment, ensuring that care plans are responsive to the unique needs and strengths of each youth and family. Key Responsibilities Provide individual, family, and group therapy to youth with behavioral health needs and their caregivers. Develop, implement, and regularly update person-centered care plans focused on restoring and enhancing functioning. Collaborate closely with youth and families to set achievable goals and action steps tailored to their priorities. Facilitate care transitions by coordinating with hospitals, residential programs, and community providers. Support appointment navigation and community reintegration, including accompaniment and travel training when necessary. Participate actively as a core member of a multidisciplinary team, contributing to care coordination and shared treatment decisions. Establish and maintain strong partnerships with schools, community agencies, medical providers, and managed care organizations. Document all services and communications in accordance with clinical, regulatory, and productivity standards. Monitor client progress toward treatment goals, evaluating and adjusting interventions as needed. Attend team meetings, clinical supervision, and professional development trainings. Provide on-call crisis coverage and referral support to youth and families after hours as assigned. Maintain current knowledge of best practices, evidence-based interventions, and culturally responsive care approaches. Ability to engage youth and families collaboratively and maintain consistent therapeutic relationships. Commitment to identifying and leveraging client and community strengths. LMSW, LCSW, LMFT, or LCAT required. Minimum of 1 year of direct experience providing clinical services to children/adolescents with serious emotional or behavioral disorders and their families. Spanish fluency strongly preferred. Proficient in Microsoft Office and capable of learning EHR systems. Must be able to travel frequently throughout Staten Island, with occasional travel to other NYC boroughs.
