They will collaborate with care teams to improve clinical outcomes, preventative care, and reduce preventable hospitalizations. Responsible for working within a multi-disciplinary clinical team to promote effective, safe, and cost-conscious drug therapy to members.**You Will**Work within a multi-disciplinary team to provide collaborative chronic disease management through direct patient care or through consultation with the primary care team (PCP and/or NP) Disease states include but are not limited to Diabetes, Heart failure, Hypertension, Asthma, COPD, Dementia Comprehensive Medication Management (CMM) Review and optimize complex medication regimens Identify drug therapy problems and recommend alternatives Monitor for efficacy, safety, adherence, and cost-effectiveness Promote utilization of CPT II & Procedural codes Complete comprehensive medication reviews for patients with specific high-risk medication concerns high risk disease states, members who are at high risk for rehospitalization, or during transitions of care including post hospital discharge Responsible for ensuring compliance with regard to operations and enhanced member/provider experience This includes review for clinical appropriateness, drugdrug interactions, adverse drug reactions, clinical efficacy, and response, reviewing corresponding labs, etc Make recommendations to the primary care team (PCP and/or NP).Transitions of Care (TOC) Perform medication reconciliation post-discharge as required Identify and resolve medication discrepancies Coordinate follow-up care and prevent high risk readmissions (COPD, HF, Falls)Chronic Disease Management Support management of diabetes, hypertension, heart failure, COPD, etc. Titrate medications per protocol under collaborative practice agreements Provide education and adherence counseling to optimize medication adherence (e.g. 90-100 day supply, medication synchronization) Quality Improvement Prioritize closure of care gaps within chronic disease, medication, or preventative care measures Contribute both clinically and operationally to achieve Part C & Part D 5 STAR rating performance Ensure coding accuracy and capture of Transitional Care Management (TRC), CPT II codes, and/or Snowmed codes within scope of pharmacy related programs/workExecute protocols regarding appropriate medication use for members, including de-prescribing (e.g. Poly-ACH, COB STAR measures) protocols and high-risk medication review to address polypharmacy and clinical appropriateness of medicationsExecute pharmacy specific initiatives across the entire membership to achieve quality outcomes Responsible for working with members, clinical care team and SCAN pharmacy team to achieve 5 Star performance on all pharmacy related star measures- medication adherence, SUPD, SPC, TRC, Medication Review, MTM, Poly/ACH and COBCollaboration & Training: Serve as a resource to providers and staff on pharmacotherapy best practices Train staff in safe medication practices and regulatory compliance Provide training to the clinical staff in association with pharmacy specific initiatives and general clinical training at a regular cadence Communicate and work collaboratively with the clinical team to ensure interventions are aligned with pharmacy based goalsMember & Provider Experience: Responsible for ensuring compliance with regard to operations and enhanced member/provider experience.Maintain professional and technical knowledge by attending educational workshops;
