Actively engage in and contribute to a positive, respectful, and productive workplace culture and atmosphere.* Screen and interview applicants/recipients, relatives, and other interested persons in the field, to determine initial and on-going eligibility for Long Term Care Services, health care, social service, diversion/transition, and protective service’s needs.

Inform clients of rights and methods of recourse.* Develop detailed, individualized case management plans for clients.

Conduct on site field reviews using OR ACCESS, CA/PS assessment tool and the laptop computer.

Provide on-going case management by coordinating the provision of services to clients according to rules, regulations, and the individual case plans.

Conduct (direct and indirect) contacts with consumers or their authorized representatives.* Experience effectively working independently, using problem solving and critical thinking skills* Experience analyzing information or situations to make good decisions within program rules and policies* Professional communication skills required to interact with consumers, community partner and co-workers* Experience preparing and maintaining written narrative documentation of actions taken* Experience interviewing to obtain information to evaluate the next steps to take in a process* Please make sure your application materials, resume and job history, are clearly outlined as this information will be used to determine your starting salary range. Some individuals may have challenges managing their emotions.* May encounter situations requiring quick decision-making to ensure the safety of oneself or others.* Employees in these positions may be required to work a flexible work schedule as a condition of employment. This is understood to mean that the hours of work may vary from day to day and could include evenings and weekends under certain circumstances. Work schedule specifics will be determined by the hiring supervisor.* Fast paced environment with constantly evolving policies and procedures* May drive a variety of different types of vehicles to remote places in a variety of terrain and possibly inclement weather.* Will come in contact with animals.* May be exposed to communicable diseases, such as flu virus, Covid-19, and MRSA.* If selected as a finalist, we will conduct a criminal history and background check.
