Job Summary

We are seeking an experienced and dedicated Director of Clinical Management to join our home care organization. The ideal candidate will be responsible for overseeing and managing all clinical aspects of our home care services, ensuring the highest quality of care for our clients while maintaining compliance with regulatory standards and organizational policies. Key Responsibilities

Clinical Leadership:

Provide strategic direction and clinical leadership to the home care team, including nurses, therapists, and other healthcare professionals. Quality Assurance:

Develop, implement, and monitor quality assurance programs to ensure the delivery of safe, effective, and patient‑centered care. Policy Development:

Create and maintain clinical policies and procedures in accordance with state and federal regulations, as well as industry best practices. Staff Management:

Oversee the recruitment, training, and ongoing development of clinical staff, fostering a culture of continuous improvement and excellence in patient care. Care Coordination:

Collaborate with interdisciplinary teams to ensure seamless care coordination and optimal patient outcomes. Performance Improvement:

Identify areas for improvement in clinical processes and implement strategies to enhance efficiency and effectiveness of care delivery. Regulatory Compliance:

Ensure compliance with all relevant healthcare regulations, including HIPAA, Medicare, and state‑specific home care requirements. Risk Management:

Develop and implement risk management strategies to minimize potential liabilities and ensure patient safety. Financial Management:

Collaborate with the finance department to develop and manage clinical budgets, optimize resource allocation, and improve cost‑effectiveness of care delivery. Technology Integration:

Evaluate and implement appropriate healthcare technologies to enhance care delivery, documentation, and communication within the home care setting. Community Relations:

Represent the organization in community events and collaborate with external healthcare providers to promote continuity of care and expand service offerings. Emergency Preparedness:

Develop and maintain emergency preparedness plans for home care clients and staff, ensuring readiness for various scenarios. Qualifications

Bachelor's degree in Nursing, Healthcare Administration, or related field required; Master's degree preferred Current RN license in good standing Minimum of 5 years of experience in home care or related healthcare setting At least 3 years of experience in a clinical management or leadership role Demonstrated knowledge of home care regulations, quality standards, and best practices Strong understanding of healthcare reimbursement models, including Medicare and private insurance Excellent leadership, communication, and interpersonal skills Proficiency in healthcare information systems and electronic health records Ability to analyze complex clinical data and make informed decisions Experience in developing and implementing quality improvement initiatives Strong organizational and time management skills Certification in Case Management (CCM) or Home Care Nursing (HCS-D)
