Position Summary The Permanency Case Manager provides comprehensive case management services with a primary focus on achieving permanency for youth and their families. This role delivers individual, family, and group interventions while working collaboratively within an interdisciplinary treatment team that may include social work, psychiatry, nursing, education, and direct care staff. The Case Manager serves as an advocate, liaison, and ambassador for youth and families, ensuring timely documentation, coordinated services, and strong community connections that promote safety, stability, and long-term success. The position actively supports transitions, aftercare planning, and permanency outcomes while promoting equitable and inclusive practices.

Therapeutic Crisis Intervention (TCI)

First Aid/CPR , provided by the agency.
Position Specific Duties/Responsibilities Provide permanency-focused case management services to assigned youth and families
Build positive, collaborative relationships with youth, families, direct care staff, counties, and community systems
Serve as an advocate for children and families within child welfare, educational, and community systems
Participate actively in multidisciplinary treatment teams to coordinate services including health care, education, family time, and community supports
Facilitate referrals and aftercare services linking youth to education, employment, housing, mental health, and permanency resources
Maintain timely, accurate, and compliant documentation including FASPs, court reports, assessments, and case records
Collaborate with Licensed Behavioral Health Practitioners to prepare reports, assessments, and supplementary documentation for Family Court and treatment planning
Attend required meetings including CSE meetings, permanency roundtables, court hearings, and case conferences
Provide direct services in office settings, family homes, and the community
Support youth and families in preparing for and maintaining consistent family time and safe transitions
Partner with families to develop, implement, and review home safety plans and court-ordered requirements
Engage youth in permanency activities including Lifebooks, Family Finding events, Heart Gallery, and adoption-related resources
Assist youth with transition planning, independent living preparation, and documentation needs (e.g., identification, credit checks, SSI, FAFSA, Access-VR)
Screen permanency resources and complete home studies for freed youth when applicable
Participate in permanency-focused trainings and may provide in-service training to multidisciplinary teams
Follow all agency, departmental, state, and federal policies and procedures
Attend mandatory supervision, training, and certification sessions
Perform other duties as assigned by immediate supervisor
Knowledge, Skills, and Abilities Strong ability to work effectively with youth, families, staff, and community partners from diverse cultural and ethnic backgrounds
Knowledge of child welfare systems, permanency planning, and community-based resources
Strong advocacy, problem-solving, and organizational skills
Ability to function effectively as part of an interdisciplinary team while maintaining professional boundaries
Effective communication skills across written, verbal, and electronic formats
Flexibility, adaptability, and ability to manage competing priorities in a dynamic environment
Qualifications Bachelor’s degree in Human Services or related field
Master’s degree in Human Services or related field (preferred)
1–2 years of related experience or training
Valid driver’s license, adequate auto insurance, and ability to meet agency driving criteria (required)
Ability to pass an annual physical per agency health policy (Category 2 classification)
