Registered Nurse‑Patient Navigator (Full Time) Days‑Farmington Hills Serves as a patient navigator throughout the continuum of care. Responsibilities:

Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.

Coordinates efforts for treatment and promotes quality improvement and educational efforts.

Compiles data, tracks outcomes, and makes recommendations for process improvement.

Provides referrals/support for follow‑up care for patients who are uninsured or underinsured.

Location:

This will be working at our Farmington Hills location but may travel to other locations in the area if needed.

Education / Experience Required

Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

Minimum of 2 years Registered Nurse med‑surg experience.

Experience with patient care plans preferred.

Comfortable with Microsoft office suite applications (Excel, Word, calendar, Outlook and Teams meetings).

Comfortable with interacting with patients both face‑to‑face and telephonic.

Comfortable with communicating with other disciplines (physicians, social workers, office staff, community health).

Comfortable with documenting in EHR as well as navigating EHRs.

Certification / Licences Required

Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required.

BLS Provider credentialed from the American Heart Association (AHA) or American Red Cross preferred.
