Registered Nurse-Patient Navigator (Full Time) Days - Farmington Hills
Location:
Farmington Hills, Michigan (may travel to other locations in the area if needed)
Shift: Monday-Friday 8:00 AM - 4:30 PM (no weekends, holidays, or on-call)

Responsibilities
Serve as a patient navigator throughout the continuum of care.
Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
Coordinate efforts for treatment and promote quality improvement and educational efforts.
Compile data, track outcomes, and make recommendations for process improvement.
Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
Education / Experience Required
Diploma from an accredited school/college of nursing OR professional licensure at time of hire.
Minimum of 2 years of Registered Nurse Med-surg experience.
Preferred Qualifications
Experience with patient care plans.
Comfortable with Microsoft office suite applications (Excel, Word, Calendar, Outlook, Teams).
Comfortable with interacting with patients both face-to-face and telephonic.
Comfortable with communicating with other disciplines (physicians, social workers, office staff, community health).
Comfortable with documenting in EHR and navigating EHRs.
Strong communication skills.
Certification / Licences Required
Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date.
BLS Provider credentialed from the American Heart Association (AHA) or American Red Cross preferred.
