Job Description
Pilsen Wellness Center, Inc. is a community‑based not‑for‑profit agency that provides culturally sensitive services to a multicultural population with limited financial resources. The organization’s mission is to provide quality human services to economically disadvantaged children and families. The agency consists of five divisions – Mental Health, Substance Abuse, Youth Services, Early Intervention, and Education – and is currently supported through 17 different locations.
Program Summary PATH (Projects for Assistance in Transition from Homelessness) is a federally funded initiative designed to connect individuals with serious mental illness (SMI) who are experiencing homelessness to mental health, substance use, housing, and support services. Services may include outreach, screening, treatment referrals, and housing assistance. The ideal candidate will work with individuals experiencing homelessness and living with serious mental illness such as schizophrenia, bipolar disorder, and major depression.
Work Schedule Monday – Friday, 9:00 am – 1:00 pm.
Position Summary This individual participates in the development and implementation of individual service plans to increase a participant’s independence skills in daily living, economic self‑sufficiency, and community integration. Services may include an array of interventions designed to provide treatment, habilitation, training, and other community integration services.
Education and Experience
Experience with field‑based outreach and engagement
Skills in crisis de‑escalation, harm reduction, and Housing First principles
Cultural humility and the ability to build trust with diverse populations
Knowledge of mental health systems, housing resources, and public benefits
Flexibility to work in shelters, encampments, and street settings
Strong documentation and collaboration skills
This individual must have a Bachelor’s degree in Psychology or a related field OR 5 years’ experience in the mental health field. Familiarity with Medicaid Rule 132 is desired. A valid driver’s license is required. Bilingual (Spanish) is required.
Compensation $22.00 USD per hour
