The Integrated Case Manager for Population Health is a vital member of the patient-centered care team, responsible for assessment, planning, care coordination, and advocacy for patients and their families. The role focuses on addressing complex health care needs following critical events or diagnoses, ensuring optimal health outcomes as patients transition through various care settings. Responsibilities
Conduct comprehensive assessments of patient and family biomedical, psychological, social, and functional needs. Develop personalized, patient-centered care plans to enhance the patient care experience. Engage patients and families through advocacy, communication, health education, and resource identification. Utilize judgment and critical thinking to help patients overcome barriers to achieving their goals. Provide counseling related to treatment decisions and end-of-life issues, including advanced care planning. Facilitate seamless patient transitions between care settings. Collaborate with the treatment team to manage patients with complex medical and social needs. Evaluate patient responses to care plans and modify as necessary. Participate in process improvement activities to reduce risk and improve service delivery. Support interventions involving vulnerable populations, such as victims of abuse. Required Experience / Certifications / Licensure
Minimum 5 years of acute inpatient case management experience, including acute discharge planning. MSW certification required. BLS certification through AHA required. Experience using CarePort system preferred. Experience charting in AllScripts and GEMS/Sunrise is a plus. Registered Nurse (RN) or Licensed Social Worker (LMSW) with an unrestricted State of Michigan license. Why ARMStaffing?
