Responsibilities
Conduct psychosocial assessments to identify barriers to treatment, plan discharge and transition services, and collaborate with social workers, nurses, physicians, case managers, and community organizations to coordinate resources for housing, financial assistance, transportation, and housing stability. Support patients and families navigating emotional stress, grief, or end‑of‑life decisions with culturally competent counseling and resource navigation. Document social histories, psychosocial needs, risk factors, and progress toward goals, contributing to quality improvement initiatives that refine care pathways. Build durable care plans, strengthen family readiness for post‑acute care, and advocate for equitable access to services. Engage in professional growth through mentorship, cross‑disciplinary learning, and exposure to diverse patient populations across hospital settings and community‑based organizations.
