About the Practice We are seeking a skilled and patient-focused Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a busy cardiology practice supporting Ronald D’Agostino, DO, a highly experienced cardiologist with extensive expertise in cardiovascular imaging and disease prevention.

Dr. D’Agostino completed both his residency and cardiology fellowship at Long Island Jewish Medical Center, where he also served as Chief Medical Resident. He is board certified in Internal Medicine and Cardiovascular Disease by the American Board of Internal Medicine and brings decades of experience in the early detection, diagnosis, and management of heart and vascular diseases, with a particular emphasis on advanced cardiovascular and valvular imaging. His approach combines the latest diagnostic technologies with highly personalized patient care.

Position Summary The APP will work collaboratively with Dr. D’Agostino in an outpatient cardiology practice, providing comprehensive evaluation, management, and follow-up care for patients with cardiovascular and vascular conditions. This role is ideal for an APP who is passionate about preventive cardiology, advanced diagnostics, and long‑term patient relationships.

Key Responsibilities

Perform comprehensive cardiovascular histories and physical examinations

Evaluate and manage patients with heart and vascular conditions under physician supervision

Assist in the interpretation and coordination of cardiovascular imaging and diagnostic testing

Provide preventive cardiology care, including risk assessment and patient education

Manage follow‑up visits, medication adjustments, and chronic disease management

Document patient encounters accurately in the electronic medical record (EMR)

Coordinate care with referring providers and ancillary services

Qualifications

Graduate of an accredited Nurse Practitioner or Physician Assistant program

Active New York State NP or PA license (or ability to obtain)

National board certification (ANCC/AANP or NCCPA)

Cardiology experience preferred; strong interest in cardiology considered

Excellent clinical judgment, communication, and interpersonal skills

Commitment to patient-centered, high-quality care
