Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, and Georgia. Responsibilities:
Support the mental and physical health of patients on an assigned caseload.
Screen, assess, and facilitate engagement for common mental health and substance abuse disorders.
Provide patient education about available treatment options.
Track treatment progress and monitor patients via in‑person or telephone encounters.
Consult for psychotropic medication management and coordinate with primary care providers.
Deliver brief evidence‑based behavioral interventions (e.g., behavioral activation, problem‑solving treatment, motivational interviewing).
Refer to evidence‑based psychosocial treatments as clinically indicated.
Participate in weekly caseload consultation with psychiatric consultants and communicate recommendations to medical providers.
Document patient progress and treatment recommendations in the EHR and required systems.
Facilitate referrals for services outside the organization (e.g., housing assistance, vocational rehabilitation).
Develop relapse prevention self‑management plans for discharged patients.
Utilize Excel, Word, and assessment trackers daily; manage time effectively through calendars and the EMR system.
Conduct telephonic and in‑person visits as a component of the role.
Qualifications:
Minimum of MSW; preferred licensure or provisional licensure.
Behavioral health experience and knowledge.
Licensed driver with an insured automobile in good working order.
Excellent verbal and written communication skills.
