RN Admission Nurse Hospice of South Texas (HOST)

Rotation: Tuesday - Friday / 10:00 AM - 8:00 PM
POSITION SUMMARY
Hospice of South Texas (HOST) is seeking a compassionate and skilled RN Admission Nurse to complete hospice admissions and provide comprehensive skilled nursing care to patients in their place of residence. This role serves as a critical clinical liaison between patients, families, referral partners, and the interdisciplinary team, ensuring timely and appropriate admission, care coordination, and regulatory compliance.
KEY RESPONSIBILITIES
Admissions & Clinical Assessment
Perform comprehensive admission assessments addressing physical, functional, psychosocial, spiritual, and environmental needs.
Review referral and admission documentation for appropriateness and eligibility.
Apply hospice admission criteria to determine level of care and patient eligibility.
Assess caregiver readiness and immediate support needs upon admission.
Establish individualized Plans of Care and recommend revisions as needed.
Care Coordination & Patient Education
Coordinate care with the interdisciplinary team (IDT), physicians, and referral partners.
Educate patients and families on disease progression, symptom management, nutrition, self-care, and end-of-life expectations.
Communicate changes in patient condition and care outcomes to appropriate team members.
Determine scope and frequency of services based on acuity and patient/family needs.
Documentation & Compliance
Complete and submit all documentation timely per HOST policies and regulatory standards.
Monitor assigned cases to ensure compliance with third-party payer requirements.
Utilize electronic systems and organizational technology for accurate communication and documentation.
Maintain infection control and standard precautions in all care activities.
Professional Practice & On-Call
Participate in assigned on-call rotation providing patient/family support.
Demonstrate professional growth, competency, and adherence to hospice philosophy.
Promote HOST administrative policies and standards of care.
Perform additional duties as assigned.
QUALIFICATIONS
Required
Graduate of an accredited Diploma, Associate, or Bachelor of Nursing program
Current Texas RN license
Valid Texas Driver's License and reliable insured transportation
CPR certification and Hepatitis consent/declination
Strong interpersonal, communication, and assessment skills
Ability to work independently in home-based settings
Preferred
Minimum two (2) years of RN experience in clinical care
Hospice or home health experience
Specialized training in palliative or end-of-life care
PHYSICAL & WORK ENVIRONMENT REQUIREMENTS
Prolonged standing, walking, and ability to lift up to 50 pounds
Ability to transfer and assist patients safely
Exposure to home environments, infectious disease risk, and varying weather conditions
Flexible scheduling with PRN emergency call responsibility
Ability to work under time-sensitive and emotionally complex conditions
