MDS Coordinator (RN)
to join our clinical leadership team.
Position Overview The MDS Coordinator plays a critical role in ensuring accurate, timely, and compliant completion of all Minimum Data Set (MDS) assessments. This position supports quality resident outcomes, regulatory compliance, and optimal reimbursement while collaborating closely with the interdisciplinary care team.
Key Responsibilities
Coordinate and complete MDS assessments in accordance with federal and state regulations
Ensure accuracy, timeliness, and compliance of all assessment and clinical documentation
Collaborate with the interdisciplinary team (IDT) to develop and maintain individualized resident care plans
Monitor resident status changes and ensure documentation reflects care and services provided
Provide education and guidance to staff on MDS processes, care planning, and regulatory requirements
Participate in quality assurance, audits, and compliance initiatives
Stay current on MDS 3.0 updates, reimbursement systems, and regulatory changes
Qualifications
Active
Registered Nurse (RN)
license in the state of Ohio (required)
Previous MDS Coordinator experience or long-term care clinical experience (preferred)
Strong working knowledge of
MDS 3.0, RAI process, and CMS regulations
Excellent organizational, analytical, and communication skills
Proven ability to work collaboratively within an interdisciplinary team
High level of accuracy and attention to detail
