Speech Language Pathologist (PRN) Pay Rate:

$55.00/hour

SUMMARY:

Provide practical established Speech Language Pathologist skills to residents following Fahrney‑Keedy’s policies and procedures.

Maximize the communicative and swallowing competence of each resident.

Provide comprehensive diagnostic, consultative, and treatment services for residents with hearing, language, speech, voice, cognitive and swallowing disorders.

Provide strong verbal and written communication skills with residents, family members, visitors, staff, etc.

DELEGATION OF AUTHORITY:

As an SLP you are delegated the administrative authority, responsibilities and accountability necessary for carrying out your assigned duties.

EMPLOYMENT AT WILL:

Employment with Fahrney‑Keedy is “at will”. Either the associate or Fahrney‑Keedy may terminate the employment relationship for any reason at any time without notice.

JOB FUNCTION:

Essential functions are identified, but this description does not exclude other duties that may be performed.

RESIDENT CARE DUTIES & RESPONSIBILITIES

Provide speech therapy screenings, evaluations, and treatments according to scope of practice.

Obtain informed consent from resident and/or POA.

Perform tasks in accordance with established policies and procedures.

Render initial evaluations and ongoing treatment as indicated; provide counseling regarding needs of Speech Therapy.

Participate in development, coordination and implementation of resident care and discharge plans.

Evaluate progress and discharge residents from SLP services as indicated.

Follow work assignments and schedules to complete assigned tasks.

Submit evaluation reports by end‑of‑treatment day, summarizing needs, treatment plans, diagnostics, goals, prognosis, and discharge plans.

Complete weekly clinical progress notes, daily treatment encounter notes, and rehab discharge programs via Optima.

Treat residents with up‑to‑date techniques following appropriate planning.

Determine and recommend SLP services per resident care plan; follow physician orders and ethical standards.

Follow established procedures of physical agent modalities.

Cooperate with inter‑departmental staff to maintain therapy services.

Create and maintain a warm, personal, positive, and calm environment.

Report complaints and grievances immediately; report absences or injuries per protocol; adhere to productivity standard of 85%.

Follow other delegated tasks as outlined on the Evaluating Therapist Task List.

ADMINISTRATIVE DUTIES

Agree not to disclose user ID/password; report violations.

Agree not to disclose resident PHI; report violations.

Report unauthorized access attempts to the supervisor.

Ensure workstations are logged off or protected by screensaver.

SAFETY & INFECTION CONTROL

Keep call buttons within easy reach of each resident.

Notify Director of Therapy of missing or unattended residents.

Follow safety precautions and keep equipment off floor.

Clean, disinfect, and return equipment after use.

Report hazardous conditions, equipment defects, safety violations, and communicable diseases.

Follow isolation precautions and use safety equipment as required.

Use only equipment trained for; practice hand hygiene.

WORKING ENVIRONMENT The work environment is moderate noise, involves frequent interruptions, and may include hostile or emotionally upset residents. May require longer hours, weekends, holidays, emergency call‑backs, hazardous chemicals, and exposure to infectious disease.

Works throughout the facility.

Subject to frequent interruptions and call‑backs.

May be required beyond normal hours and on weekends/holidays.

Exposure to hazardous chemicals and infectious waste.

Attends educational programs.

Subject to injury from falls, burns, odors, dust, disinfectants, and other air contaminants.

RESIDENT RIGHTS

Maintain confidentiality of resident care information.

Treat residents fairly with kindness, dignity, and respect.

Ensure nursing care is provided in privacy.

Knock and wait for permission before entering rooms.

Report grievances and complaints to Delegating RN.

Report abuse or misappropriation to Abuse Coordinator.

Honor resident refusal of treatment; report to supervisor.

SPECIFIC REQUIREMENTS

Maintain current licensure.

Maintain CPR and First Aid certification.

Attend continuing education required by law.

Participate in program development, in‑service training, and educational programs in SLP.

Provide education to Rehab Department following CEU events.

Require proficiency in reading, writing, speaking, and understanding English.

Demonstrate knowledge and skills for age‑related resident needs.

Support teamwork and be a positive team member.

Make independent decisions when warranted.

Treat staff, residents, families, visitors, government entities, and public tactfully.

Work harmoniously with other staff.

Possess patience, tact, cheerfulness, and enthusiasm; handle difficult residents.

Seek out and incorporate new methods and principles.

Relate information about resident condition effectively.

Do not pose direct threat to health and safety of others.

EDUCATION

Master’s Degree in Speech Language Pathology; certification of Clinical Competence.

Licensure in applicable state.

PHYSICAL & SENSORY REQUIREMENTS

Move intermittently throughout the workday.

Speak and write English understandably.

Manage mental and emotional stress effectively.

Have functional vision and hearing or adequate prosthetics.

Function independently with integrity and effective teamwork.

Meet general health requirements, including medical and physical examination.

Relate to ill, disabled, elderly, emotionally upset, and at times hostile individuals.

Assist in evacuation of residents during emergencies if necessary.
