PRN Physical Therapist (PT)
to join our
in-house rehabilitation team
in Houston, TX. This opportunity is ideal for a clinician who is passionate about patient-centered care and enjoys flexible scheduling in a collaborative skilled nursing environment.

Position Summary As a Physical Therapist, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services to support functional recovery. You will collaborate closely with the interdisciplinary team to ensure consistent, compassionate care.
Key Responsibilities
Evaluate residents’ physical conditions and functional limitations
Develop and implement individualized treatment plans using evidence-based practices
Provide skilled physical therapy interventions to improve strength, balance, mobility, and function
Monitor resident progress and adjust treatment plans as needed
Educate residents, families, and caregivers on exercises and safety strategies
Complete accurate and timely documentation per facility and regulatory guidelines
Participate in interdisciplinary meetings and care planning
Maintain compliance with all state, federal, and facility policies
Qualifications
Active Texas Physical Therapist (PT) license
In good standing with all licensing boards and regulatory agencies
SNF or long-term care experience preferred
Familiarity with Net Health documentation system a plus
Strong communication and interpersonal skills
