Location: Northwest Ohio

A respected, community-based nonprofit health system in Northwest Ohio is seeking a compassionate and experienced Nurse Practitioner to join its Palliative Care team. This full-time role involves both inpatient and outpatient palliative care services and is ideal for a provider who is passionate about improving quality of life for patients facing serious or terminal illness.

Position Highlights:
  • 75% Inpatient / 25% Outpatient Palliative Care

  • Based at a leading regional hospital in Lima, OH

  • Manage patient consultations focused on:

    • Symptom management

    • Chronic disease management

    • Advanced care planning

    • Hospice transition support

  • Lead sensitive conversations with patients and families regarding prognosis and goals of care

  • Conduct inpatient rounds, including ICU coverage for palliative/hospice patients

  • Outpatient visits occur across oncology clinics, infusion centers, health clinics, and via telehealth

  • Collaborate with ICU, Oncology, Cardiology, Pulmonology, and other specialists

  • Independently manage care in a fast-paced hospital setting with support from a regional palliative team

  • Document care in Meditech (inpatient) and eClinicalWorks (outpatient)

  • Educate hospital staff as a clinical expert in palliative care

  • Average 6 patient encounters per day , Monday Friday

Candidate Requirements:
  • Board Certified Nurse Practitioner REQUIRED

  • Active Ohio license (or willingness to obtain) REQUIRED

  • Significant experience in acute care and/or palliative/hospice settings REQUIRED

  • Clean malpractice and license history REQUIRED
