Family Medicine Physician

A strong community health system in New York City is seeking a Primary Care Physician to join their team. About the Opportunity: Schedule: Monday-Friday, 1 Saturday/Sunday per month Hours: 9am-5pm, one later night per week Setting: Outpatient Patient population: All ages Responsibilities: Provide comprehensive primary care for patients of all ages in an outpatient community health setting Perform routine physicals, preventive care, chronic disease management, and acute visit evaluations Support underserved and diverse populations with a focus on access, equity, and continuity of care Collaborate with behavioral health, care management, dental, and specialty referral teams Maintain accurate documentation and follow quality metrics, compliance standards, and value-based care goals Requirements: License to practice medicine in New York Board Certified/Eligible in Family Medicine Interest in community health and working with diverse populations
